Are you concerned about your ability to live independently? Take the Independent Living Assessment today!
What is the Independent Living Assesment (ILA)?
The ILA, powered by Boston University Activity Measure for Post Acute Care (AM-PAC) and sponsored by Philips Lifeline, is a free, computerized evaluation taken online which assesses your ability to live independently.
Why should I take the ILA?
Living independently is an important lifestyle choice. Taking the ILA can help you understand what you are capable of managing independently and where you may need some support. The ILA is designed to assist you in developing a plan to accomodate changes in your health and function without losing your independence.
What can I expect from the assessment?
Taking the assessment is fast and easy. You will be asked to answer a series of questions regarding your abilities. After you complete the survey, the responses are scored and interpreted and an individualized report is generated and e-mailed to you. The report gives a score for each of the three functional areas and describes the implications of the score for independent living.
How do I take the assessment?
The Independent Living Assessment can be accessed easily by using a web browser and visiting www.lifeline-ILA.com.
